Before your visit:
- Please bring all insurance cards to your initial visit.
- Please bring a current medication list with dosages and start date.
- Please do not apply lotion, oil, or powder to the arms, legs and chest. (this may interfere with the EKG leads)
Follow-up patients please inform ACHC staff of any changes to the following when scheduling:
- Insurance provider
- Address / Phone number
- Email address
- Primary doctor
*Please arrive 15 minutes prior to your visit. If you are running more than 15 minutes late to your scheduled appointment, the ACHC staff reserves the right to reschedule the appointment, as Dr. Patel likes to have enough time with patients and family members to understand the diagnosis and instructions. If you are unable to make your scheduled appointment time, please call the ACHC staff at least 24 hours prior to the appointment time. (Cancellation or No Show fees may apply)
